The results are encouraging, though not yet conclusive. We observed a reduction in readmissions over the twelve-month period, and we intend to confirm it in a larger cohort before drawing firm conclusions. The data were reviewed independently, and the limitations are set out in the appendix. We would welcome comment from colleagues on the methods, particularly the handling of missing records. A full report will follow once the second review is complete.
